FDA and USDA FSIS have published eight separate Class II drug recalls for compounded injectable products manufactured by Carolina Infusion LLC of Ridgeland, South Carolina. All recall notices were posted on 10 September 2026.[1] Recalled products include multiple strength formulations of semaglutide, tirzepatide, and combinations of each agent with cyanocobalamin. All products are supplied in single-use dosage syringes. All eight notices identify the same deficiency: lack of assurance of sterility. Regulators linked the sterility concern to use of bacteriostatic water in product formulation, and to processing equipment used for the compounded items. Dispensed volume for each product is calculated based on the specific patient prescription.
