An analysis of the J-DOIT3 randomised controlled trial has found no clear lower threshold of glycated hemoglobin associated with the lowest risk of all-cause mortality or myocardial infarction in people with type 2 diabetes. The study was published in PLoS One and listed on PubMed on 23 October 2026.[^1]

The trial followed 1271 participants over 8.5 years.

Authors reported no HbA1c threshold for minimum mortality was identified across the observed range. The analysis did find maintaining LDL cholesterol below 80 mg/dL was associated with reduced stroke risk.
